Missouri Driving Records (Form 4569)
A Missouri driving record is the DOR's official summary of a driver's license history, points, suspensions, and traffic convictions. The Missouri Department of Revenue uses Form 4569 for printed requests and offers online access through the DOR portal. Records come in certified and uncertified versions and cover different lookback windows depending on the product ordered.
Types of Missouri driving record
The DOR issues three driving record products:
- 3-Year Driver Record: the most common option for personal review and basic insurance verification — covers the past 36 months of moving violations, suspensions, and points.
- 5-Year Driver Record: the standard for most employer screenings and court-ordered submissions — includes the past 60 months of activity.
- Lifetime (Complete) Driver Record: the full history from license issuance, including original license issuance, name and address history, and any permit, intermediate, or commercial endorsements ever held.
All three products can be ordered as certified or uncertified. A certified copy carries the DOR seal and is required for most court proceedings, commercial driver hiring decisions, and certain insurance underwriting cases.
How to order online
The fastest way to obtain a Missouri driving record is through the DOR's online portal. Log in with your Missouri driver's license number, the last four of your SSN, and your date of birth. Select the record type, pay the fee by card, and download a PDF immediately for the uncertified version. Certified copies are mailed within 5 to 7 business days.
How to order by mail or in person
To order by mail or at a fee office, complete Form 4569 with:
- Your full legal name and any prior names (maiden, married, hyphenated).
- Date of birth and full Social Security number.
- Missouri driver's license number.
- Mailing address for the printed record.
- Reason for request (your own record, court order, employer, attorney).
- Signature and date.
Mail the form with payment to the DOR Driver License Bureau in Jefferson City, or submit it at any fee office. Fee office processing is same-day for uncertified records; certified records arrive in 5 to 10 business days.
Fees
Missouri driving record fees:
- $2.82 for an online uncertified 3-year record (the lowest among most states).
- $7.39 for a certified driver record.
- $1 additional fee for online processing.
Fees can be paid by check, money order, or card. Cash is accepted only at fee offices, not for mail orders.
Third-party requests
Missouri follows the federal Driver's Privacy Protection Act (DPPA): a third party (employer, insurance company, attorney) cannot obtain another person's driving record without a permissible purpose and, in most cases, the driver's signed consent. Common permissible purposes include employment verification (for jobs that involve driving), insurance underwriting, court proceedings, and licensed investigative services. Employers in trucking, ridesharing, delivery, and other driving-required positions typically use the DOR's commercial driver record retrieval service rather than the personal Form 4569.
Disputing an entry
If your Missouri driving record contains an entry you believe is incorrect — a conviction that did not occur, a suspension that was already lifted, or a point assessment from another driver — submit a written dispute to the DOR with supporting documentation:
- A certified court disposition showing dismissal or amendment of the underlying citation.
- A reinstatement letter or order showing a suspension has been cleared.
- An affidavit from another party if the incident is mistakenly attributed to you.
The DOR reviews disputes within 30 days. If the agency cannot verify the requested correction, the next step is to file a petition in the county circuit court that handled the underlying matter. The DOR updates the record after the court issues a corrected disposition.
Active points roll off the Missouri driving record one year after the conviction date for most non-DWI offenses, with progressive reductions on the 2nd and 3rd anniversaries.
